If you are a returning patient please download, print, and fill out only the Health History Form and the Index form that correlates to your area of injury.
Don't forget to bring your filled out forms with you to your first visit. If unable to print please come in 15 minutes early to fill out forms.

What is an Index Form and how do I fill it out?
This questionnaire form has been designed to give your therapist information as to how your pain has affected your ability to manage everyday life. Please answer every question by placing a mark in the circle that best describes your condition today.
What should I wear to Physical Therapy?
You should dress comfortably with access to your area of injury or pain. You should also be wearing comfortable shoes. Gym clothes are usually the best clothes to wear. There are rooms for you to change in as needed.
How long should I plan on being there?
Each visit lasts approximately 60 minutes where you will be one-on-one with your therapist.
Do you accept health insurance and will you bill my Insurance Company for my treatment?
As a courtesy to you, we will verify your outpatient physical therapy benefits and submit claims to your insurance company, but these are not a guarantee of benefits/payment and ultimately you are responsible for all charges. Please call our office for more details.
Do I need a prescription to come to physical therapy?
That depends on your specific insurance plan; however, certain providers like Medicare will always require one. Most physical therapy prescriptions are good for 30 days.
